General ICU
Overview
In 2006, the general ICU was established. Presently, the department has an energetic and harmonious team, including 3 chief physician, 2 deputy chief physicians; 1 deputy chief nurse, 22 supervisor nurses. Among them, there are 1 master supervisor, 2 doctors and 9 masters. With 40 beds, it accommodates more than 3,000 severely ill patients per year.
The department is equipped with advanced medical equipment, including advanced vital sign central monitoring system, extracorporeal membrane oxygenation(ECMO) instrument, continuous renal replacement therapy device, blood perfusion instrument, fiber bronchoscope, NICOM and PICCO cardiac output monitor, bedside ultrasound machine, 24-hour dynamic EEG monitor, to ensure that critically ill children can receive the best quality monitoring, treatment and rescue. It is at the domestic leading level in the fields of pediatric cardiopulmonary resuscitation, various types of shock, multiple injuries, severe sepsis, severe pneumonia, ARDS, fulminant myocarditis, various poisonings, and multiple organ failure.
The department successfully created the “National Youth Civilization” in 2014. As a standing director unit of the Emergency Department of the Chinese Pediatrics Society, it undertook the domestic training of pediatric intensive care personnel, maintained technical exchanges with many domestic and foreign hospitals, participated in international pediatric advanced life support training courses, and successfully organized domestic pediatric advanced life support training.
(1) Leading diagnosis and treatment technology at home and abroad: mechanically assisted breathing (including neurally adjusted ventilatory assist, high-frequency oscillatory ventilation), PICOO cardiac output monitoring technology,extracorporeal membrane oxygenation, temporary cardiac pacing technology, continuous blood purification, plasma exchange, blood perfusion, immunoadsorption, peritoneal dialysis,bronchoalveolar lavage technique, tracheotomy, metabolic and nutritional support in critically ill patients, cardiopulmonary and cerebral resuscitation after cardiopulmonary arrest, mild hypothermia, 24-hour brain function monitoring, arterial blood pressure monitoring, central venipuncture, and pleural cavity puncture.
(2) Diagnosis and treatment of various critical and severe diseases: Severe sepsis, severe pneumonia (including ventilator-associated pneumonia), ARDS, severe asthma, severe hand-foot-mouth disease, fulminant myocarditis, severe congenital heart disease with heart failure, various severe arrhythmias, severe hepatitis with liver failure, severe pancreatitis, severe encephalitis, brainstem lesions, Reye's syndrome, severe Guillain-Barre syndrome, hemophilia syndrome, and various types of shock (especially septic shock and anaphylactic shock), and various accidental injuries (trauma, drowning, poisoning), multiple organ failure (respiratory failure, heart failure, hepatic failure, kidney failure, etc.), various serious electrolyte disorders, etc.
Professional Features
The department is one of the largest, most fully equipped and technologically advanced professional pediatric critical care rescue monitoring centers in China. It is at the domestic advanced level in the fields of pediatric cardiopulmonary resuscitation, various types of shock, severe sepsis, severe pneumonia, ARDS, and acute poisoning, especially in the application of extracorporeal membrane lung treatment for refractory cardiopulmonary failure, reaching the international leading level. With a rescue rate of more than 60%, it has been successfully used to treat children with fulminant myocarditis and severe anaphylactic shock.
Our center is one of the earliest children's hospitals in China that uses continuous blood purification technology. The comprehensive application of different blood purification technologies has successfully rescued a large number of critically ill children with diseases such as acute renal failure, acute liver failure, acute pulmonary edema, severe sepsis, severe hand-foot-mouth disease, severe allergic diseases and various severe poisonings.
In recent years, infectious epidemic diseases have continued to erupt, such as SARS, avian influenza, hand-foot- mouth disease, etc., the center is responsible for the rescue and consultation of critically ill patients with epidemic diseases in Zhejiang Province. In the past 5 years, it has successfully treated more than 140 cases of critical hand-foot-mouth disease and more than 60 cases of severe avian influenza. In 2011, the successful rescue of Niuniu, who fell from the tenth floor to the ground,moved the whole country, and was named one of the ten most touching examples of beautiful angels in the health system of Zhejiang Province.
The clinical research scope of the department involves the treatment of severe sepsis, rapid detection of pathogens of severe respiratory infections, clinical early warning of severe respiratory syncytial virus infection, comprehensive diagnosis and treatment of critical hand-foot-mouth disease, and pediatric ECMO critical treatment, etc. It has been awarded 2 National Natural Science Funds, 1 major scientific research project of Zhejiang Provincial Science and Technology Department, 1 Zhejiang Provincial Natural Science Foundation, and many other bureau-level projects, and has applied for more than 3 million project funds.
At present, the department cooperates with well-known hospitals in Shanghai, Nanjing, Hunan, Wenzhou and other places to carry out multi-center research on CRRT technology intervention in children's sepsis. In the past 5 years, the center has participated in the completion of 2 books and published dozens of high-quality clinical research papers, including more than 10 SCI papers (the highest impact factor is 4.0).
As one of the technology conversion teams for the diagnosis and treatment of major children's diseases in Zhejiang Province, it holds the annual meeting of the Pediatric Emergency Department of Zhejiang Province, the national “Pediatric Advanced Life Support”, “Pediatric Blood Purification” and “Pediatric Intensive Care Specialist Nurses” training courses, receiving more than 300 students from inside and outside of the province, while undertaking the training of visiting doctors from various regions.
